PHC - Social Worker Bright Horizons In-Patient

Responsibilities:

Complies with PENN HIGHLANDS CLEARFIELD policies and procedures, accreditation agency requirements, and federal, state or local law and regulations

1. Conducts patient assessments and completes a psychosocial summary which includes assessing home environment. Offers assistance in resolving issues that impact on patient care.

a. Performs psychosocial evaluation within the policy time frame.

b. Performs a thorough evaluation /assessment on each patient which encompasses their cognitive functioning, current functional ability in their home setting, home environment and safety issues, financial problems/matters, coping mechanisms, family/SO involvement in care, need for community services, paid caregivers or possible nursing/personal care home placement.

c. Documents all information on psychosocial evaluation and submits timely as per policy.

d. Evaluates all data and recommends referrals as needed.

2. Develops appropriate treatment plans as per the patient s diagnosis utilizing the information on history and physical completed by the physician and initial and psychosocial assessments.

a. Develops realistic and measurable goals.

b. Implements and reviews treatment plans with patients.

c. Completed the treatment planning process timely as per policy.

d. Updates treatment plans timely as per policy.

e. Participates in interdisciplinary treatment planning conferences.

3. Provides treatment coordination with assigned patients.

a. Assists patients and their families with personal, emotional, environmental and family difficulties that interfere with obtaining maximum benefit from mental health treatment.

b. Provides or refers out family therapy as deemed necessary by self and other treatment team members.

c. Reviews with patients their responses to events and interactions since last visit.

d. Effectively utilizes knowledge of age appropriate needs and abilities in the assessment, treatment and education of patients and families.

e. Completes documentation for each service provided and collateral contacts made in a timely manner.

4. Facilitates individual, group and family therapy.

a. Assists patients in developing coping skills to deal with physical changes, cognitive losses, emotional and behavioral changes, and lifestyle changes.

b. Documents patient s group participation and individual and family therapy sessions daily in the record as per policy.

5. Develops appropriate discharge plans.

a. Formulates discharge plan in a timely manner as per policy.

b. Maintains files of resources available in the community

6. Keeps communication lines open, participates in staff/team conferences to help coordinate efforts with members of the treatment team.

a. Participates in staff conferences/communication to help coordinate and promote good patient care.

b. Documents on 100% of all patient clinical records the case conference information shared with any appropriate staff members/physicians/community resources.

c. Keeps an open line of communication with the Program Director and Unit Manager via memo, phone and personal contact to work together to resolve patient problems, and to consult together regarding documentation, utilization, and agency information.

7. Completes mandated hospital training programs and attends appropriate seminars when able.

a. Attends the yearly mandatory in-service and 75% of the monthly staff meetings and other assigned committee meetings.

b. Attends educational seminars as appropriate to maintain licensure.

General duties as directed.

Qualifications

Education:

Social worker with a Master s degree from a school of social work accredited school by the Council on Social Work Education and current Pennsylvania licensure preferred. Master s Degree in other counseling discipline and licensure as a professional counselor also acceptable.

The fraudster will send a check to the victim who has accepted a job. The check can be for multiple reasons such as signing bonus, supplies, etc. The victim will be instructed to deposit the check and use the money for any of these reasons and then instructed to send the remaining funds to the fraudster. The check will bounce and the victim is left responsible.
